Penelitian terbaru: Vegan di rekomendasikan untuk penyakit jantung

In this guest blog, Kim A. Williams, MD, a cardiologist at Rush University in Chicago and the next president of the American College of Cardiology, explains why he went vegan and now recommends it to patients.

Physicians want to influence their patients to make lifestyle changes that will improve their health, but sometimes the roles are reversed and we are inspired by patients. It was a patient’s success reversing an alarming condition that motivated me to investigate a vegan diet.

Just before the American College of Cardiology’s (ACC) annual meeting in 2003 I learned that my LDL cholesterol level was 170. It was clear that I needed to change something. Six months earlier, I had read a nuclear scan on a patient with very-high-risk findings — a severe three-vessel disease pattern of reversible ischemia.

The patient came back to the nuclear lab just before that 2003 ACC meeting. She had been following Dean Ornish, MD’s program for “Reversing Heart Disease,” which includes a plant-based diet, exercise, and meditation. She said that her chest pain had resolved in about 6 weeks, and her scan had become essentially normalized on this program.

When I got that LDL result, I looked up the details of the plant-based diet in Ornish’s publications — 1- and 5-year angiographic outcomes and marked improvement on PET perfusion scanning — small numbers of patients, but outcomes that reached statistical significance.

I thought I had a healthy diet — no red meat, no fried foods, little dairy, just chicken breast and fish. But a simple Web search informed me that my chicken-breast meals had more cholesterol content (84 mg/100 g) than pork (62 mg/100 g). So I changed that day to a cholesterol-free diet, using “meat substitutes” commonly available in stores and restaurants for protein. Within 6 weeks my LDL cholesterol level was down to 90.

I often discuss the benefits of adopting a plant-based diet with patients who have high cholesterol, diabetes, hypertension, or coronary artery disease. I encourage these patients to go to the grocery store and sample different plant-based versions of many of the basic foods they eat. For me, some of the items, such as chicken and egg substitutes, were actually better-tasting.

There are dozens of products to sample and there will obviously be some that you like and some that you don’t. One of my favorite sampling venues was the new Tiger Stadium (Comerica Park) in Detroit, where there are five vegan items, including an Italian sausage that is hard to distinguish from real meat until you check your blood pressure — vegan protein makes blood pressures fall.

In some parts of country and some parts of world, finding vegan restaurants can be a challenge. But in most places, it is pretty easy to find vegan-friendly options with a little local Web searching. Web searching can also help with the patients who are concerned about taste or missing their favorite foods. I typically search with the patient and quickly email suggestions.

Interestingly, our ACC/American Heart Association (AHA) prevention guidelines do not specifically recommend a vegan diet, as the studies are very large and observational or small and randomized, such as those on Ornish’s whole food, plant-based diet intervention reversing coronary artery stenosis. The data are very compelling, but larger randomized trials are needed to pass muster with our rigorous guideline methodology.

Wouldn’t it be a laudable goal of the American College of Cardiology to put ourselves out of business within a generation or two? We have come a long way in prevention of cardiovascular disease, but we still have a long way to go. Improving our lifestyles with improved diet and exercise will help us get there.

1 lbr daun salam
1 cm lengkuas
1 sdm kecap manis
1 sdm saos tiram vegan
4 buah cabai merah keriting
3 buah cabai hijau
2 sdm minyak sayur
100 ml air
gula pasir

Cara mambuat :

• Putren di siangi dan iris serong.
• Baso vegan di iris menjadi tiga, beri irisan garis-garis miring di masing-masing permukaan.
• Lengkuas di memarkan.
• Cabai merah dan cabai hijau di iris serong.
• Panaskan minyak, tumis irisan cabai, daun salam dan lengkuas hingga harum.
• Masukkan putren dan baso vegan, tumis hingga layu.
• Tuangi air. Masukkan kecap manis, saos tiram vegan, garam dan gula.
• Tambahkan kecambah, aduk rata. masak kembali hingga matang. angkat dan sajikan.

Bayam Renyah Sambal kencur

1 ikat sayur bayam
3 sdm tepung beras
2 sdm tepung terigu
130 cc air
minyak (untuk goreng)

• Sayur bayam dipetik ambil bagian daunnya saja, cuci dengan bersih. tiriskan.
• Campur tepung beras, tepung terigu dan garam jadi satu.
• Tambahkan air sambil diaduk hingga menjadi cairan tepung.
• Panaskan minyak di wajan. setelah minyak cukup panas, kecilkan apinya.
• Ambil beberapa helai sayur bayam, celupkan ke adonan tepung lalu masukkan dalam minyak panas.
• Goreng hingga berwarna kuning keemasan, angkat. titiskan.

High Cholesterol Increases Risk of Breast Cancer

High cholesterol levels increase the risk for breast cancer, according to a study presented at the Frontiers in Cardiovascular Biology Meeting last week in Barcelona, Spain.

Researchers observed 664,159 women in the U.K. from the Algorithm for Comorbidities, Associations, Length of Stay and Mortality (ACALM) study. Risk of developing breast cancer was 64 percent higher in women with high cholesterol. This study builds upon previous research that linked obesity to breast cancer risk.

Background: Vegetarian diets might affect the risk of cancer.

Objective: The objective was to describe cancer incidence in vegetarians and nonvegetarians in a large sample in the United Kingdom.

Design: This was a pooled analysis of 2 prospective studies including 61,647 British men and women comprising 32,491 meat eaters, 8612 fish eaters, and 20,544 vegetarians (including 2246 vegans). Cancer incidence was followed through nationwide cancer registries. Cancer risk by vegetarian status was estimated by using multivariate Cox proportional hazards models.

Results: After an average follow-up of 14.9 y, there were 4998 incident cancers: 3275 in meat eaters (10.1%), 520 in fish eaters (6.0%), and 1203 in vegetarians (5.9%). There was significant heterogeneity between dietary groups in risks of the following cancers: stomach cancer [RRs (95% CIs) compared with meat eaters: 0.62 (0.27, 1.43) in fish eaters and 0.37 (0.19, 0.69) in vegetarians; P-heterogeneity = 0.006], colorectal cancer [RRs (95% CIs): 0.66 (0.48, 0.92) in fish eaters and 1.03 (0.84, 1.26) in vegetarians; P-heterogeneity = 0.033], cancers of the lymphatic and hematopoietic tissue [RRs (95% CIs): 0.96 (0.70, 1.32) in fish eaters and 0.64 (0.49, 0.84) in vegetarians; P-heterogeneity = 0.005], multiple myeloma [RRs (95% CIs): 0.77 (0.34, 1.76) in fish eaters and 0.23 (0.09, 0.59) in vegetarians; P-heterogeneity = 0.010], and all sites combined [RRs (95% CIs): 0.88 (0.80, 0.97) in fish eaters and 0.88 (0.82, 0.95) in vegetarians; P-heterogeneity = 0.0007].

Conclusion: In this British population, the risk of some cancers is lower in fish eaters and vegetarians than in meat eaters.

Minum obat diabetes/ gula harus minum vitamin B12, mengapa ?

Studi terbaru menunjukkan bahwa, metformin  mampu menurunkan kadar vitamin B12 yang diperkirakan berkontribusi terhadap penurunan fungsi kognitif pada pasien DM. Hal ini merupakan kesimpulan dari studi yang dilakukan oleh Dr. Moore dan kolega yang telah dipublikasikan secara online dalam jurnal Diabetes Care bulan Oktober 2013.

Hampir 30 % pasien yang mendapatkan metformin mengalami defisiensi vitamin B12 dan ini diduga adanya interaksi antara pemberian metformin dan reseptor pada bagian distal ileum yang menghambat ambilan dari vitamin B12.  Hasil penelitian ini dikritisi untuk tidak menjadi peringatan  tetapi mendapatkan perhatian dalam memonitor kebutuhan dan mengkoreksi akan kadar vitamin B12.

Dalam satu penelitian yang dipimpin oleh Eileen M dari departemen bedah universitas  Deakin University, RS.Geelong Australia didapatkan hasil yang cukup menarik bagi para peneliti dan memerlukan penelitian lanjutan, yaitu didapatkan kaitan pemberian Metformin pada pasien diabetes tipe 2 yang mengalami gangguan fungsi kognitif akibat defisiensi vitamin B12 dan suplementasi kalsium. Mengenai hal ini sebenarnya sejak tahun 1970 klinisi sudah cukup peduli melihat adanya kaitan antara pemberian metformin dan penurunan dari kadar vitamin B12 dalam plasma,

Penelitian dengan analisis final mencakup 1.354 pasien, 104 adalah pasien diabetes tipe 2, 22 pasien dengan gangguan toleransi glukosa,  dari 126 pasien disglisemia 35 pasien mendapatkan metfomirn dan 91 tidak, partisipan berusia 51-99 tahun (mean 74 tahun), dan 59,5% adalah wanita, dimana penilaian MMSE (Scores on the Mini-Mental State Examination) digunakan dalam menilai kemampuan kognitif dimana skor normal adalah 28-30 sekitar 50%, gangguan minimal skor 24-27 didapatkan sebesar 27% dan gangguan sedang skor 18-23 didapatkan sebesar 18% dan gangguan berat dengan skor kurang dari 18 didapatkan sebesar 10%.

Setelah penyesuaian usia, jenis kelamin, edukasi dan tingkat depresi, subjek DM tipe 2 dengan performance kognitif buruk yaitu MMSE < 28 tanpa diabetes didapatkan odd rasio 1,51 (p : 0,033) dan pada kelompok diabetes dengan performance kognitif buruk yang mendapatkan metformin didapatkan odd rasio 2,23,  dengan skor MMSE adalah 22,8 pada yang mendapatkan metformin dibandingkan 24,7 pada pasien diabetes yang tidak mendapatkan metformin. Diantara pasien diabetes dengan skor MMSE rendah memiliki kadar vitamin B12 kurang dari 250 pmol/L dibandingkan pasien dengan kadar vitamin B12 lebih tinggi (MMSE 22,9 vs 25).

Dari hasil penelitian ini, peneliti merekomendasikan bagi klinisi agar pasien diabetes harus dapat dipertahankan kadar vitamin B12 secara adekuat yaitu lebih dari 300pmol/L untuk menurunkan risiko penurunan fungsi kognitif. (ARI)

Six Tips for Preventing Cancer

Researchers with the Physicians Committee have published six dietary guidelines for cancer prevention in the Journal of the American College of Nutrition. Just in time for picnic and grilling season, the guidelines recommend eliminating hot dogs and avoiding red meat and instead throwing veggie kabobs and other plant-based foods on the grill. They also recommend eliminating dairy products and limiting alcohol.

The six dietary recommendations to reduce risk of several types of cancer are:

1. Limit or avoid dairy products to reduce risk of prostate cancer.
One glass of milk each day increases risk of prostate cancer by 10 percent. Consuming two glasses of milk each day increases risk of prostate cancer by 60 percent.

2. Limit or avoid alcohol to reduce cancers of the mouth, pharynx, larynx, esophagus, colon and rectum, skin, and breast.
One drink per week increases risk of mouth, pharynx, and larynx cancers by 24 percent. Two to three drinks per day increase risk of colorectal cancer by 21 percent.

3. Avoid red and processed meat to reduce risk of cancers of the colon and rectum.
Each 50-gram daily serving of processed meat, equivalent to two slices of bacon or one sausage link, increases risk of colorectal cancer by 21 percent.

4. Avoid grilled, fried, and broiled meats to reduce risk of cancers of the colon, rectum, breast, prostate, kidney, and pancreas.
Certain heterocyclic amines (HCAs) are associated with cancer of the colon and rectum. HCAs form in cooked skeletal muscle, increasing with higher cooking times and higher temperatures. When ingested, HCAs can disrupt DNA synthesis.

5. Consume natural soy products, such as edamame, to reduce risk of breast cancer.
A global study shows women who consume 11 grams of soy protein each day reduce risk for both premenopausal and postmenopausal breast cancer by about 30 percent.

6. Eat a variety of fruits and vegetables to reduce several forms of cancer.
The fiber and phytochemicals available in fruits and vegetables, especially leafy greens, help reduce overall cancer risk—while a Western diet (high amounts of meat and fat with minimal amounts of fruits and vegetables) doubles

Plant-Based Diet Reverses Heart Disease

A new research report confirms that heart disease can be dramatically improved—and even reversed—by a plant-based diet.

Researchers from this study counseled 198 patients with cardiovascular disease on a diet free of fish, meat, dairy, and added oils. Of the 89 percent of participants who followed the diet, 81 percent improved their symptoms and experienced fewer complications from heart disease.

In addition, those participants lost an average of 18.7 pounds, while 22 percent saw a complete reversal of their condition. This study employed a nutritional training program that eliminated both added oils and animal products.
